Dietitian Consultation Agreement

DIETITIAN CONSULTATION AGREEMENT

PARTIES This agreement is entered into between ………………………………. (hereinafter referred to as the CLIENT ) and ………………………………. (hereinafter referred to as the DIETITIAN ) under the following terms and conditions

SUBJECT
MATTER 1 – The subject matter of this agreement is to regulate the scope of personalized nutrition programs and consulting services to be prepared by the Dietitian in accordance with the Client's nutritional and lifestyle goals.

DIETITIAN'S RESPONSIBILITIES
ARTICLE 2 –
2.1. Prepares a personalized nutrition program taking into account the client's age, weight, height, health status, and lifestyle.
2.2. All recommendations are based on scientific principles and comply with current legislation.
2.3. Keeps confidential the information provided by the client regarding their health history and does not share it with third parties.
2.4. Provides regular follow-up sessions and updates to the client.

CLIENT'S OBLIGATIONS
ARTICLE 3 –
3.1. The client is obligated to provide accurate and complete information about their health status.
3.2. The client is responsible for following the program prepared by the dietitian, but acknowledges that it does not substitute for treatment.
3.3. The client shall fully fulfill their payment obligations.

FEES AND PAYMENT
ARTICLE 4 –
4.1. The client will pay a consultancy fee of ……… TL + VAT each month.
4.2. Payment for package programs is made in advance.
4.3. Payments are made within … days from the invoice date.

DURATION
ARTICLE 5 – The contract commences on …/…/…. and is valid for a period of … months. Unless either party gives notice of termination, it will be renewed under the same conditions.

CONFIDENTIALITY
ARTICLE 6 – The dietitian is obligated to protect the client's health information in accordance with the Personal Data Protection Law (KVKK).

LIMITATION OF LIABILITY
ARTICLE 7 – The dietitian is not liable for any consequences arising from the non-implementation of the recommended program or from the client's false statements.

TERMINATION
ARTICLE 8 – If either party fails to fulfill its obligations, the other party may terminate the contract by giving written notice.

AUTHORIZED COURT
ARTICLE 9 – In case of disputes arising from this contract, the Courts and Enforcement Offices of …………… shall have jurisdiction.
