TO THE DUTY FAMILY COURT
OF ANKARA

PLAINTIFF:
Name and Surname – (Turkish Republic Identity Number: …) – Address

ATTORNEY:
Lawyer's Name and Surname – Address – Telephone Number

DEFENDANT:
Name and Surname – Address

SUBJECT: Request for authorization to represent the company.

EXPLANATIONS:

  1. Our client, …, and the defendant, …, were married on …/…/… and they have a daughter named …, born on …/…/….
  2. Our child, …, has qualified to compete in the … World Cup. The … Federation has decided to select our child for the national team. Our child, who will represent our country in the competition to be held in … city on …/…/…, must travel abroad no later than …/…/….
  3. For a child to travel abroad and represent our country, the joint consent of both spouses is required. However, the defendant spouse is refusing to allow their child to travel abroad and represent our country in the competition without providing any justification. During a meeting with my client, the defendant stated, "If you file for divorce and provide a written statement that you will not appeal the decision, I will take the necessary steps for the child to participate in the competition and obtain their passport.".
  4. The joint child, who previously participated in similar competitions in countries such as Hungary, Bulgaria, and Slovenia on behalf of the defendant spouse, is unable to obtain a new passport because its validity has expired. The defendant spouse does not consent to the joint child obtaining a passport. The client, who wants the joint child, who has been working and dedicating himself to this field since approximately the age of four, to realize his dream of participating in the competition, believes that this competition, which is under state protection and will contribute to the child's development, should request the court's intervention in order to unilaterally give permission (regarding the passport application).
  5. My client is employed at … Hospital and is entitled to obtain a green passport for his daughter to travel abroad as specified. Furthermore, he is capable of handling the necessary procedures for the child. For these reasons, we request that the court authorize my client to represent the child in matters including passport procedures, limited to the scope of the specified competition.

EVIDENCE:

  1. Population records,
  2. Document for sending abroad,
  3. Information and documents relating to the achievements of the joint child,
  4. Other legal evidence.

LEGAL GROUNDS: Law No. 4721, Article 188; Law No. 4787, Article 4; and other relevant legislation.

CONCLUSION AND REQUEST: For the reasons briefly explained above,

  1. We request that your esteemed court accept our rightful claim and, due to the defendant's unjustifiable refusal to consent, authorize our client to represent us in the necessary procedures, including obtaining a passport, for our joint child's participation in the aforementioned competition, in lieu of the defendant's consent

I respectfully submit this request.
